If you're researching ketamine-assisted psychotherapy (KAP), you're probably feeling a mix of curiosity and nervousness about your first session — that's completely normal. KAP has become one of the most promising options for facing treatment-resistant depression, anxiety, PTSD, and OCD, but knowing what to expect can make the difference between feeling anxious and feeling ready. Here's an evidence-based guide to preparing for your first session.

What Is Ketamine-Assisted Psychotherapy?

KAP combines low, sub-anesthetic doses of ketamine with structured psychotherapy delivered before, during, and after the medicine session. Unlike ketamine infusion clinics that focus purely on the drug's pharmacological effects, KAP intentionally pairs the dosing experience with a licensed therapist's support, using the temporary reduction in psychological defenses to help clients process trauma and emotional material more directly (Dore et al., 2019). A large, multi-site effectiveness study following 1,806 adults found significant and lasting reductions in depression, anxiety, and PTSD symptoms at three and six months post-treatment, with roughly half to three-quarters of participants reporting clinically meaningful improvement (Yermus et al., 2023).

How to Prepare for Your First KAP Session

1. Complete Your Medical and Psychiatric Screening

Before your first dosing session, your prescriber will review your medical history, current medications, and any contraindications such as uncontrolled hypertension or certain psychiatric conditions. Full transparency here isn't just a formality — it's what keeps the experience safe (Sanacora et al., 2017).

2. Set a Clear (but Flexible) Intention

Therapists often recommend anchoring your first session with a simple, open-ended intention rather than a rigid goal — something like "I want to understand where my anxiety comes from" instead of a specific outcome you're determined to reach. Research on psychedelic-assisted treatments consistently shows that this kind of preparatory intention-setting shapes how meaningfully a session unfolds (Dore et al., 2019).

3. Plan Your Practical Logistics

  • Fast lightly beforehand, per your clinic's specific guidance, to reduce nausea risk.

  • Wear comfortable clothing and skip contacts if you tend to keep your eyes closed for extended periods.

  • Arrange a ride home. Ketamine affects coordination and judgment for several hours; you should not drive yourself.

  • Clear your schedule for the rest of the day so you're not rushing into work or errands afterward.

4. Protect Your "Set and Setting"

The psychedelic research concept of "set and setting" — your internal mindset and your external environment — has direct clinical relevance to KAP outcomes. Clinics intentionally create calm, low-stimulation spaces (soft lighting, eye shades, curated music) because the psychological and environmental context of the experience shapes how it's processed (Dore et al., 2019). On your end, this means avoiding stressful conversations, doomscrolling, or work deadlines in the 24 hours leading up to your appointment.

5. Talk Openly With Your Therapist Beforehand

Your preparation session is the time to voice any fears — about losing control, about the dissociative effects, about what might surface emotionally. A good KAP therapist will walk you through what the experience typically feels like physically (a sense of floating, altered perception of time, mild dissociation) so nothing catches you off guard.

What to Expect Emotionally

Many clients describe their first session as introspective rather than dramatic — more like a quiet inward journey than a jarring experience. Some sessions bring up strong emotion or unexpected memories; others feel more meditative. Both are considered clinically useful, and your therapist is present throughout to help you stay grounded.

Integration: The Work Continues After the Session

The days following your dose are considered a window of heightened neuroplasticity, when the brain is especially receptive to forming new patterns of thought and behavior (Dore et al., 2019). Integration sessions — where you and your therapist unpack what came up — are where the insights from the medicine session get translated into lasting change. Skipping integration is one of the most common reasons people don't get the full benefit of KAP.
